Job Summary: Frontier Home Health is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide skilled, patient-centered care in the home setting. In this role, you’ll work closely with an RN and interdisciplinary team to support patients in achieving better health outcomes—all from the comfort of their homes.

Essential Functions

• Provide skilled nursing care under the supervision of an RN and in accordance with the plan of care
• Administer medications and treatments as ordered
• Perform nursing interventions within LPN scope of practice
• Assist with procedures while maintaining proper safety and infection control standards
• Monitor and document patient responses to care
• Promote a safe, high-quality care environment in the home
• Collaborate with RNs, physicians, therapists, and other care team members
• Adapt care based on patient needs and condition changes
• Support compliance with care plans and regulatory standards
• Complete accurate and timely clinical documentation (within 48 hours)
• Report changes in patient condition to the supervising RN promptly
• Participate in case conferences and care planning discussions
• Educate patients and caregivers on care plans and health management
• Attend trainings, in-services, and case conferences
• Maintain clinical competency through continuing education
• Participate in quality improvement initiatives
  
  

Requirements

• Active and unrestricted LPN license in Colorado
• Knowledge of basic nursing care and medication administration
• Strong communication and teamwork skills
• Ability to work independently and manage time effectively
• Basic computer skills and EMR experience preferred
