- Administer medications and treatments as ordered
- Perform nursing interventions within LPN scope of practice
- Assist with procedures while maintaining proper safety and infection control standards
- Monitor and document patient responses to care
- Promote a safe, high-quality care environment in the home
- Collaborate with RNs, physicians, therapists, and other care team members
- Adapt care based on patient needs and condition changes
- Support compliance with care plans and regulatory standards
- Complete accurate and timely clinical documentation (within 48 hours)
- Report changes in patient condition to the supervising RN promptly
- Participate in case conferences and care planning discussions
- Educate patients and caregivers on care plans and health management
- Attend trainings, in-services, and case conferences
- Maintain clinical competency through continuing education
- Participate in quality improvement initiatives
- Active and unrestricted LPN license in Colorado
- Knowledge of basic nursing care and medication administration
- Strong communication and teamwork skills
- Ability to work independently and manage time effectively
- Basic computer skills and EMR experience preferred
Salary : $32 - $40
