We are seeking a Registered Nurse in Colorado Springs, CO to provide high-quality, patient-centered care in the home setting. In this role, you will manage patient care plans, perform skilled nursing visits, and support patients in maintaining independence and safety at home.
Key Responsibilities
Skilled Nursing & Care Assessment
- Provide skilled nursing visits in accordance with the established plan of care.
- Perform comprehensive patient assessments (physical, psychosocial, and environmental).
- Monitor patient condition and identify changes in health status.
- Deliver age-appropriate care in alignment with nursing standards.
- Collaborate with physicians, therapists, and interdisciplinary team members.
- Report changes in condition and adjust care plans as needed.
- Communicate effectively with patients, families, and caregivers.
- Ensure continuity of care across all services.

- Educate patients and families on disease management, medications, and treatments.
- Provide guidance on safety, infection prevention, and home care techniques.
- Encourage patient engagement and adherence to the care plan.
- Complete timely and accurate documentation in the EMR system.
- Follow all agency policies, safety protocols, and infection control standards.
- Maintain compliance with HIPAA and regulatory requirements.
- Participate in trainings, case conferences, and quality improvement initiatives.
- Active and unrestricted RN license required.
- 1 year of nursing experience required, home health experience preferred.
- Valid CPR certification.
- Current driver's license, reliable transportation, and auto insurance.
- Strong clinical judgment and assessment skills.
- Excellent communication and time management abilities.
- Ability to work independently in a home-based setting.
- Commitment to maintaining professionalism and patient confidentiality.
- Prolonged standing, walking and driving required.
- Ability to lift, move or assist patients and equipment up to 50 pounds.
- Ability to work under stressful conditions to meet clinical and documentation deadlines.
- Possesses manual dexterity, hand-eye coordination, and physical stamina.
- Ability to assess patient needs, make timely decisions, and address psychosocial needs of patients and families.
