Role Description
The MDS Coordinator is a Part-time, on-site position located in Dublin, OH. The role involves overseeing and managing the Minimum Data Set (MDS) assessment process to ensure compliance with state and federal regulations. Day-to-day responsibilities include completing accurate resident assessments, developing individualized care plans, and collaborating with interdisciplinary teams to optimize care for residents. The MDS Coordinator will also monitor and ensure proper coding for resident assessments to support financial reimbursement and quality care standards.
Qualifications
- Active licensure as a Licensed Practical Nurse (LPN) or an RN
- Proficiency in Care Planning and Care Plans development
- Comprehensive knowledge and experience with MDS processes and requirements
- RAC-CT Certification Preferred
- Strong background in Geriatric Nursing
- Attention to detail and ability to ensure regulatory compliance
- Solid communication and collaboration skills for working with interdisciplinary teams
- Experience with electronic health record (EHR) systems is preferred
- Bachelor's degree in Nursing or related healthcare field is a plus
