We are seeking a dedicated and compassionate Field License Practical Nurse (LPN) to provide high-quality, patient-centered care in a community setting. Duties
- Perform thorough patient assessments, including vital signs, medication administration, and physical examinations to inform care plans.
- Assist with medical procedures such as catheterization, sterile processing, and wound suturing while adhering to aseptic techniques.
- Manage medication administration accurately using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems, ensuring compliance with HIPAA regulations.
- Provide specialized care for patients with complex needs such as dementia, Alzheimer's disease, or those requiring hospice or palliative care.
- Support patients with developmental disabilities or those in assisted living and long-term care settings by delivering personalized nursing interventions.
- Conduct patient education on health coaching topics including nutrition, medication management, and chronic disease management.
- Collaborate with multidisciplinary teams to coordinate discharge planning, case management, and post-acute care services.
- Utilize physiology knowledge and diagnostic evaluation skills to monitor patient status and respond promptly to emergent situations such as acute pain or airway management needs.
Experience
Candidates should demonstrate extensive experience working in diverse healthcare environments including hospital medicine, ICU (Intensive Care Unit), emergency medicine, or trauma centers such as Level I or Level II trauma centers. Prior experience with medical-surgical units, geriatrics, pediatrics—including neonatal ventilator care—and home & community care is highly valued. Familiarity with EMR/EHR systems like Cerner or Athenahealth is preferred. Additional desirable experience includes:
- Critical care experience and ICU experience
- Medical office experience and triage skills
- Long-term care and nursing home experience
- Emergency room or urgent care settings familiarity
- Knowledge of ICD-9/ICD-10 coding for medical documentation
- Experience with infection control practices and sterile procedures
- Skills in phlebotomy, injection administration, specimen collection/processing, and ventilator management
