About the Role:
**THIS IS A FULL-TIME, IN-PERSON POSITION**
The Rehabilitation Specialist plays a critical role in supporting individuals recovering from physical, mental, or emotional challenges to regain their independence and improve their quality of life. This position involves assessing clients' needs, developing individualized rehabilitation plans, and coordinating with healthcare professionals to implement effective treatment strategies. The specialist will work closely with PRP Counselors to provide guidance, supervision, and support throughout their rehabilitation journey, ensuring progress is monitored and goals are adjusted as necessary.
Minimum Qualifications:
- Bachelor’s degree in Rehabilitation Counseling, Social Work, Psychology, or a related field.
- Relevant certification or licensure (i.e. CPRP, LMSW, LCSW, etc.).
- Experience working with diverse populations in rehabilitation or therapeutic settings.
- Strong understanding of rehabilitation principles, therapeutic techniques, and client-centered care.
- Excellent communication and interpersonal skills to effectively engage clients and collaborate with healthcare teams.
Preferred Qualifications:
- Certification as a Certified Rehabilitation Counselor (CRC) or equivalent credential.
- Experience with case management and familiarity with community resources and support services.
- Proficiency in electronic health records (EHR) systems and data management.
Responsibilities:
- Conduct comprehensive assessments of clients' physical, emotional, and social needs to develop individualized rehabilitation plans.
- Collaborate with multidisciplinary teams including therapists, and social workers to coordinate care and optimize treatment outcomes.
- Provide supervision for PRP workers who conduct therapeutic interventions and support to clients, adapting approaches based on progress and feedback.
- Educate clients and their families on rehabilitation processes, coping strategies, and community resources to promote sustained recovery.
- Maintain detailed documentation of client progress, treatment plans, and communications to ensure continuity of care and compliance with regulations.
Skills:
The Rehabilitation Specialist utilizes strong assessment and analytical skills daily to evaluate client needs and develop tailored rehabilitation plans. Effective communication and empathy are essential for building trust with clients and their families, facilitating motivation and adherence to treatment. Collaboration skills enable the specialist to work seamlessly with multidisciplinary teams, ensuring comprehensive care delivery. Organizational and documentation skills are critical for maintaining accurate records and tracking client progress over time. Additionally, proficiency with technology, including electronic health records, supports efficient management of client information and coordination of services. This is a supervisory position.

Salary : $60,000 - $75,000
