At Frasier, we’re seeking a compassionate, dependable Community Life Coordinator – Memory Support to design and lead person-centered programs for residents living with dementia. You’ll bring patience, creativity, and consistency to daily experiences—supporting dignity, comfort, and quality of life through meaningful engagement.
Location: Boulder, CO (On-site)
Schedule: Tuesday–Saturday | 8:30 AM–5:00 PM (evenings/holidays as needed)
Pay Range: $21.05–$26.15/hour (based on experience and qualifications)

In this role, you’ll plan and facilitate engaging experiences that help residents feel calm, connected, and supported. Responsibilities include:
- Plan and facilitate daily group, small-group, and one-to-one programs for residents living with memory loss
- Create a calm, predictable environment that supports comfort, dignity, and engagement
- Design sensory-based and purpose-driven activities tailored to resident abilities and preferences
- Collaborate closely with nursing and caregiving staff to align programming with care plans and routines
- Develop and share monthly activity calendars
- Track participation and complete required documentation accurately and on time
- Partner with families to gather life history and preferences
- Support resident transitions and foster routine and familiarity
- Experience in senior services, memory support, or a related field (preferred)
- Experience leading activities/programs with older adults (strongly preferred)
- Clear, respectful communication with residents, families, and team members
- Strong organization, attention to detail, and comfort adapting in the moment
- Comfort working evenings/weekends as program needs require
- High school diploma required; Bachelor’s degree preferred
- Ability to speak, read, and write English
