WHAT YOU WILL DO
Collaborate with senior and departmental leaders to set priorities and develop strategies for advancing community benefit initiatives throughout the system in accordance with system and federal requirements.
Develop and implement quality assurance and quality improvement activities to ensure effective interventions and stewardship of resources.
Manage the triennial required Community Health Needs Assessments (CHNA) for the individual hospitals (IRS Form 990-Section H) and create and update the related strategic implementation plans, annual reporting and updating.
Provide support for the CHNAs and developing dashboards for the implementation strategies, providing strong analytical support for all community health programs supported by the hospitals.
Develop and facilitate training of the community benefit regulation, community health data trends, and associated content to internal staff to ensure compliance with the community benefit regulation.
Support community initiatives to promote collaborative arrangements that address the unmet health needs of the community; participates in or coordinates involvement of other leadership in key community nonprofit organizations, committees, task forces and initiatives addressing identified community health.
Monitor public and population health trends and activities of appropriate organizations such as the Association for Community Health Improvement (ACHI).
Work with the Franciscan Alliance Accountable Care Organization (ACO)s, Foundation, and Mission leaders in providing support in epidemiology, community benefit knowledge and tracking, and to develop mutually beneficial goals.
WHAT IT TAKES TO SUCCEED
Prior experience in a supervisor role, as well as prior experience in public health and community engagement.
Strong analytical and interpretive skills.
Experience in program evaluation and experience in networking and building community relationships. Familiarity with Lyons CBISA Plus software and understanding of statistical software.
Excellent communication and organizational skills.
Qualifications
- Required Bachelor's Degree
- Preferred Master's Degree
- 1 year Supervisory experience Required
- 3 years Public Health/Community Engagement Required
