As a vital member of the Health and Housing Transition (HHoT) Team at Fourth Street Clinic, the Medical Case Manager plays a key role in transforming lives for those within Magnolia Apartments. This dynamic position involves assisting patients and families in navigating the maze of medical, social, educational, and other supportive services necessary for their well-being. Located in a permanent supportive housing program for individuals and families with a history of chronic homelessness, you'll be at the forefront of making meaningful connections.

Experience in medical case management or care coordination, particularly with homeless or vulnerable populations, will enhance your ability to make a significant impact.
