Responsibilities:
- Develop and implement personalized nutrition care plans for residents and patients
- Conduct comprehensive nutrition assessments and monitor progress
- Collaborate with interdisciplinary teams to ensure holistic care
- Educate residents, families, and staff on nutrition-related topics
- Maintain accurate and detailed records of patient care
- Participate in quality improvement initiatives to enhance nutrition care
- Must be a Registered Dietitian (RD) or Eligible to Become a Registered Dietitian (RD)
- Strong understanding of nutrition principles and practices
- Excellent communication, interpersonal, and organizational skills
- Ability to work effectively in a fast-paced environment
- Familiarity with electronic medical records (EMRs) and nutrition software
