Milton Nursing & Rehab Center is seeking a compassionate and motivated PRN Physical Therapist to join our dedicated rehab team! This is a great opportunity for a PT who enjoys flexibility, values a supportive team culture, and takes pride in delivering high-quality care in a SNF/LTC setting.

Responsibilities

• Evaluate, plan, and provide skilled physical therapy services to residents
• Develop individualized treatment plans to improve mobility, function, strength, and independence
• Document patient progress and treatment according to company, state, and federal standards
• Collaborate closely with PTAs, interdisciplinary team members, and nursing staff
• Educate residents, families, and caregivers on exercises, safety, and functional goals
• Deliver care aligned with best clinical practices and facility policies
