- Collaborate with nursing, rehab, dietary, and interdisciplinary teams
- Develop individualized treatment plans and functional goals
- Complete timely and accurate documentation in Net Health and PCC
- Support both short-term rehab and long-term care populations
- Current Pennsylvania SLP license
- CCC preferred
- SNF/LTC experience preferred, but not required
- Strong communication and organizational skills
