In This Role, You Will
Education and Experience:
AA Degree from an APTA accredited program. Licensure where required by law.
Two (2) years’ experience in a health care setting preferred. Outpatient experience is required.

In This Role, You Will
- Responsible for carrying out Care Plan as designed by the Physical Therapist (PT). Assist the PT during evaluations.
- Monitor resident’s progress and communicate with PT.
- Maintain documentation in system of resident’s activities, record progress and document billing activities. Perform Resident’s Home Assessment as part of the discharge process as directed by the PT.
- Perform Resident’s screening during the admission process and quarterly & annual screening to determine the need for therapy as directed by the PT.
Education and Experience:
