Job Overview
We are seeking a compassionate and dedicated Certified Home Health Aide to provide exceptional in-home care to clients with diverse needs. The ideal candidate will have a strong background in patient care, including experience with seniors, individuals with disabilities, and those requiring specialized care such as Alzheimer's or dementia. The position requires adherence to all health and safety regulations, including HIPAA compliance, and involves working closely with healthcare teams to ensure comprehensive patient care.
Duties
- Provide direct patient care including assisting with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support
- Monitor and observe patients’ health status, recording vital signs and reporting any changes promptly
- Assist with medication administration following prescribed guidelines and maintaining accurate medical documentation
- Support patients with memory care needs, including Alzheimer's and dementia care, utilizing specialized techniques
- Operate Electronic Health Record (EHR) systems to document care activities accurately and securely
- Utilize assistive devices such as Hoyer lifts for safe patient transfers and mobility assistance
- Perform catheter care, wound care, and hospice support as required
- Prepare meals according to dietary restrictions and assist with feeding when necessary
- Conduct light housekeeping tasks including laundry and cleaning to maintain a safe environment
- Follow all protocols related to HIPAA regulations to ensure patient confidentiality at all times
- Collaborate effectively with healthcare professionals and family members to coordinate comprehensive home & community care
Qualifications
- Valid Certified Home Health Aide certification required
- Proven experience in home health, long-term care, assisted living, or nursing home settings preferred
- Knowledge of medical terminology, medication administration, and medical records management
- Experience working with individuals with developmental disabilities or disabilities requiring specialized caregiving such as Alzheimer’s or dementia care
- CPR certification required; additional certifications such as hospice care or catheter care are a plus
- Ability to work independently while following detailed care plans and protocols
- Strong communication skills for working effectively with patients, families, and healthcare teams
- Compassionate attitude with a genuine desire to improve clients’ quality of life
- Ability to work flexible hours including live-in employment options if applicable
