Job Summary
We are seeking a compassionate and dedicated Certified Home Health Aide to provide high-quality in-home care to clients with diverse needs. The ideal candidate will have experience in senior care, dementia and Alzheimer’s care, and working with individuals with disabilities. This role involves assisting clients with daily activities, ensuring their safety, and supporting their overall well-being within a home or community setting. Duties
- Provide assistance with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support
- Monitor and observe patients’ health status, including vital signs and overall condition
- Assist with medication administration following proper protocols and documentation
- Support patients with memory care needs, including Alzheimer’s and dementia care strategies
- Operate and update Electronic Medical Records (EMR/EHR systems) accurately and securely in compliance with HIPAA regulations
- Assist with medical procedures such as catheter care, Hoyer lift transfers, and hospice-related tasks
- Prepare nutritious meals and assist with meal planning tailored to individual dietary needs
- Perform light housekeeping duties including cleaning, laundry, and maintaining a safe environment
- Document patient care activities thoroughly in medical records and reports
- Collaborate with healthcare teams to ensure comprehensive patient care plans are followed effectively
Requirements
- Valid certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA)
- Proven experience in home health, long-term care, assisted living, or nursing home settings
- Knowledge of medical terminology, medication administration, and medical documentation practices
- Experience working with seniors, individuals with developmental disabilities, or those requiring hospice or memory care
- Ability to work independently while maintaining excellent communication skills
- CPR certification is required; additional certifications such as Alzheimer's or dementia training are preferred
- Familiarity with EMR/EHR systems and HIPAA compliance standards
- Compassionate attitude with strong patient observation skills and attention to detail
- Ability to assist with mobility devices like Hoyer lifts safely and effectively
