Skilled Care – After‑Hours RN Coordinator
Position Overview
Under the direction of the Chief of Staff, the After‑Hours RN Coordinator is responsible for managing a designated patient population (“Clinical Team”) during after‑hours operations. This role ensures the timely, appropriate, and cost‑effective delivery of clinical services, with a strong focus on preventing avoidable hospital readmissions and maintaining continuity of care.
The After‑Hours RN Coordinator works closely with Skilled Care staff to facilitate smooth transitions from referral to admission and to provide responsive clinical and operational support outside of standard business hours.
Key Responsibilities
- Clinically triage incoming calls, texts, and emails from patients, families, and clinicians
- Dispatch clinicians after hours to address patient care issues and prevent re‑hospitalization
- Monitor the e‑charge system to accept and fully process new patient referrals
- Schedule admissions, coordinate with clinicians, and adjust next‑day schedules as needed
- Verify payor eligibility and obtain initial authorizations
- Review referrals for appropriateness for home care services and homebound status
- Introduce the agency to new patients for after‑hours admissions
- Troubleshoot clinician staffing issues that arise after hours
- Conduct Hospital Readmission Reduction Program wellness checks
Qualifications & Skills
- Active, unencumbered Registered Nurse (RN) license in the state of residence
- Graduate of an approved school of nursing
- BSN preferred, but not required
- 3–5 years of transferable experience; home health care management experience strongly preferred
- Advanced knowledge of the home health intake process
- Expert understanding of Medicare and Joint Commission (JCAHO) home health regulations
- Familiarity with office procedures, systems, and equipment
- Strong written and verbal communication skills in English
- Excellent customer service and problem‑solving skills
- Preferred: direct experience working with Joint Commission standards and Medicare regulations
Schedule & Work Structure
Monday–Friday
- 4:30 PM – 8:00 PM:
On‑site at your desk processing workflow, completing referrals, and handling tasks assigned by Operations teams - 8:00 PM – 9:00 AM:
On‑call from home, available by company‑provided desk phone only- Must remain near the phone at all times
- Cell phones are not permitted
- You may not leave the house while on duty
Weekends (Every Other Saturday & Sunday) and 4 of 8 Company Holidays
- 9:00 AM – 5:00 PM:
On‑site at your desk processing workflow and assigned operational tasks - Remainder of the day:
On‑call from home under the same requirements as weekday after‑hours coverage
Salary : $70,000
