Transportation Driver

Position Title: Transportation Driver- Part-Time (Tuscaloosa)

Department: Social Services

Reports To: Director of Social Services– Tuscaloosa

Position Purposes: The Transportation Driver is responsible for safely and reliably transporting clients to ensure access to medical appointments at Five Horizons Health Services (FHHS). This position works collaboratively with Social Services and Clinical departments to support timely and consistent transportation for clients.

Hours: This is a part-time position, and hours will vary each week according to client transportation needs. Work is completed between the business hours of 8:00 am- 5:00 pm.

Essential Functions:

· Schedule, confirm, and directly transport clients to ensure they attend medical appointments at FHHS.

· Collaborate with Medical and Social Services staff to assess and prioritize the urgency of transit needs for clients.

· Maintain clean interior and exterior of vehicle(s) used for passenger transport and report any mechanical problems to the supervisor promptly.

· Ensure all clients have a current transportation waiver on file and comply with all traffic and safety regulations during transport.

· Maintain accurate client records, document services provided, and complete required paperwork in accordance with HIPAA, Ryan White, state guidelines, and FHHS policies.

· Perform any other department or agency-related duties or special projects as directed by the supervisor.

· Participate in team meetings as needed to share updates on client transportation needs.

· Report vehicle maintenance and insurance issues to the supervisor promptly.

Education/Professional:

· Possess a valid Driver License

· Must be at least 25 years of age

· Must confirm effective auto-insurance coverage if utilizing a vehicle for work-related reasons

· Candidates must fulfill the driving record criteria set by FHHS

· Ability to interact and provide respectful services to diverse groups and cultures

Safety: When providing direct transportation services, each client must remain seated and secure according to vehicle safety laws established by the Department of Transportation (DOT). All speed limits, road signs, and signal aids must be adhered to in effort to safe transportation for clients.

Physical Requirements: This position requires an individual who is self-motivated and able to independently complete assigned responsibilities. Moreover, this individual must also be able to accept proper supervision and work cooperatively with clients, peers, and other professionals.

Frequent sitting, some standing / walking, and occasionally lifting supplies / equipment up to 50 lbs. Occasional reaching, bending, stooping, kneeling and crouching. This position requires an individual who is mentally alert and able to effectively communicate verbally and in writing. Individual must be able to effectively utilize

Microsoft Office programming and, after necessary training, electronic health records system. Occasional evening or weekend hours may be required for special events or patient needs.

Benefits: This is a part-time position and is not benefits eligible.

Pay: From $21.00 per hour
