Position Summary:
The Per Diem Home Care Nurse provides assessments, care plan, recommendations, revaluations to clients in their homes on an as-needed basis. This role supports flexible scheduling and fluctuating caseload needs by delivering patient-centered care, clinical assessments, and documentation in accordance with home health regulations and agency standards.
Reporting to: Operations Manager, Crystal Amidon
Essential Responsibilities
- Conduct in-home patient assessments and skilled nursing visits
- Develop and update individualized plan of care based on clinical findings
- Educate clients and caregivers on disease management, treatments, home safety, and preventative care as well as provide caregiver trainings
- Document visits accurately and promptly in the EMR or required system
- Communicate changes in patient condition or your clinical recommendation to the office
- Coordinate with office administrative staff
- Ensure safe, sanitary, and compliant in-home care assessment
- Respond to urgent staffing needs, new admissions, and same-day visit requests as needed
- Maintain confidentiality in alignment with HIPAA
- Active RN or LPN license (state specific)
- Current BLS/CPR
- Minimum 1 year nursing experience (home health preferred)
- Valid driver’s license & reliable transportation
- Strong clinical assessment & critical thinking skills
- Ability to work independently in the field
- Prior home care or home health experience
- Wound care certification or experience
- Experience with geriatric or chronically ill populations
- Per diem (as needed)
- Flexible availability; hours vary by client/census demand
- Weekend or holiday availability may be requested
- Ability to drive between patient homes
- Extended standing, walking, bending
