Schedule: 1st shift (8am-4:30)

In this role, you'll support individuals with disabilities in achieving greater independence and community integration by assisting with daily living skills, personal goals, and meaningful activities.

What You'll Do
- Assist individuals with daily living tasks such as hygiene, cooking, cleaning, and errands
- Support clients in achieving personal goals and participating in community activities
- Provide transportation to appointments and events
- Follow and document progress on Person Centered Service Plans (PCSPs)
- Maintain clear communication with supervisors and team members
What You'll Need
- High school diploma or equivalent
- Valid driver's license, clean driving record, and proof of auto insurance
- Flexibility to work varied shifts and fill in as needed
