DESCRIPTION: The Case Manager will provide direct, trauma-informed case management services at FPCH, and South Hayward Parish (SHP). This includes community outreach, housing placement, stabilization support, employment navigation, and coordination of wraparound services to ensure long-term stability for unhoused individuals and those at risk of homelessness.
RESPONSIBILITIES:
- Case Management
- Provide ongoing case management and support to FPCH and SHP unhoused guests and program participants, including regular follow-up meetings to monitor progress toward employment and housing stability.
- Assist individuals in obtaining identification, vital documents, benefits, employment, housing, and healthcare.
- Develop individualized service plans in collaboration with guests based on their needs, strengths, and goals.
- Advocate for guests with external agencies, landlords, and service providers to reduce barriers to stability.
- Be a proactive participant in the Case Management team.
- Tiny Home Transitional Housing Management
- Provide case management for participants of the Tiny Home Transitional Housing program, including the application, move-in, and move-out processes.
- Resource Coordination
- Connect individuals to appropriate community resources, including mental health services, substance abuse treatment, legal aid, vocational training, and housing programs.
- Maintain a current directory of community resources and services.
- Documentation & Reporting
- Maintain accurate and timely documentation in case management systems and databases (such as HMIS or Oasis).
- Track individual progress and outcomes; update service plans as needed.
- Prepare reports as required by funders, supervisors, or regulatory agencies.
- Collaboration & Communication
- Work closely with shelter staff and partner programs to ensure coordinated care and a supportive environment.
- Attend case conferences, staff meetings, and community partnership meetings.
- Act as a liaison between staff, guests, and external service providers.
- Attend and actively participate in case management meetings hosted by the City or County to stay current on housing policies and local resources.
- Proactively network with other organizations and government agencies to expand the referral network and resource pool for program participants.
- Crisis Intervention & Support
- Provide crisis intervention and conflict resolution as needed.
- Support individuals experiencing emotional distress, trauma, or urgent personal issues.
- Kinara Outreach & Daily Meaningful Activity
- Attend at least one Kinara Daily Meaningful Activity per week (such as community beautification or service events) alongside participants to foster rapport and support social integration.
- Collect and manage activity metrics and impact paperwork during community activities to track participant contributions.
- Conduct weekly internal outreach at FPCH and South Hayward Parish (SHP) to identify and engage potential participants.
- Perform external community outreach at least once a month to engage individuals in the broader community who may require services.

QUALIFICATIONS
- High school diploma, or equivalent.
- Experience in case management, preferably in a housing or homelessness services setting.
- Experience with providing outreach and successful engagement to a diverse population, including working with clients, developing trust, and the ability to respond appropriately to crisis situations.
- Knowledge of housing resources, tenant rights, and local social services.
- Strong organizational and time management skills.
- High level of integrity with the ability to maintain confidential information.
- Ability to maintain professional boundaries and engagement skills with a challenging population and in non-traditional work conditions.
- Ability to participate in outdoor community activities and outreach in various weather conditions.
- Computer experience, including troubleshooting and problem solving; proficiency in MS Office, G Suite, and data entry/management systems.
PREFERRED QUALIFICATIONS
- Bachelor’s degree in Social Work, Human Services, Psychology, or a related field (Master’s degree preferred).
- Bilingual (especially Spanish, Mandarin, or ASL).
- Familiarity with trauma-informed care and harm reduction models.
- Experience working with populations with mental health or substance use disorders.

While performing the duties of this job, the employee is regularly required to sit, stand, walk, and travel to various community locations and partner sites. The employee must be able to use standard office equipment and communicate effectively in person and by telephone. The employee will participate in outdoor community activities and outreach in varying weather conditions. Incumbents may be exposed to individuals experiencing emotional distress, trauma, or health challenges, as well as non-traditional working environments including shelters, transitional housing, and outdoor settings.
