First Choice Home Health & Hospice is seeking a skilled and compassionate Orthopedic Physical Therapist to join our home health team in Orem, UT. This role focuses on providing high-quality rehabilitative care to patients recovering from orthopedic conditions, surgeries, injuries, and mobility limitations in a home-based setting.
You will work independently while collaborating with nurses, physicians, and care coordinators to ensure effective patient recovery and improved mobility outcomes.
Key Responsibilities
- Evaluate patients’ physical condition and develop personalized treatment plans
- Provide in-home orthopedic rehabilitation therapy
- Assist patients recovering from fractures, joint replacements, sprains, and surgeries
- Improve mobility, strength, balance, and functional independence
- Document patient progress and update care plans regularly
- Educate patients and families on exercises and recovery techniques
- Collaborate with physicians and home health staff for coordinated care
- Ensure compliance with state and federal healthcare regulations
Qualifications
- Doctor of Physical Therapy (DPT) or equivalent degree
- Active Physical Therapy license in Utah (required)
- Experience in orthopedic or home health therapy preferred
- Strong clinical assessment and communication skills
- Ability to work independently in a home-care environment
- Reliable transportation for in-home visits
