Home Health Physical Therapist (PT) – East Valley

Pay: $75–$125 per visit mileage reimbursement

First Choice Home Health is seeking a compassionate, skilled Physical Therapist to join our growing home health team in the Phoenix and East Valley area. Service Area
Phoenix, Chandler, Gilbert, Mesa, Tempe, and surrounding East Valley communities.

What You’ll Do
•Evaluate patients in the home setting and develop individualized treatment plans
•Provide skilled physical therapy interventions to improve mobility, strength, balance,
safety, and function
•Educate patients, families, and caregivers on exercises, mobility strategies, and fall
prevention
•Monitor progress toward goals and modify treatment plans as needed
•Collaborate with physicians, nurses, therapists, and office staff to coordinate care
•Complete accurate and timely documentation
•Support patients in achieving the highest possible level of independence at home

Pay & Benefits
•$75–$125 per visit, depending on visit type

Qualifications
•Active Arizona Physical Therapist license
•Valid driver’s license
•Reliable transportation
•Ability to travel throughout assigned territory
•Strong clinical, communication, and documentation skills
Preferred
•Home health experience
•Residence in Phoenix or the East Valley, including Chandler, Gilbert, Mesa, or nearby communities
