Position Summary

First Call Home Health is seeking a compassionate and experienced Hospice Chaplain to provide spiritual and emotional support to hospice patients and their families. As part of our interdisciplinary hospice team, you will help patients find comfort, peace, and meaning during the end-of-life journey while honoring each person's beliefs, values, and traditions.

This role works closely with nurses, social workers, physicians, volunteers, local clergy, and community organizations to provide compassionate, patient-centered care.

Essential Duties

  • Provide spiritual and emotional support to hospice patients and their families.
  • Complete spiritual assessments and develop individualized care plans.
  • Offer prayer, spiritual counseling, religious rituals, or other support based on each patient's wishes.
  • Help patients and families navigate end-of-life decisions and advance care planning.
  • Provide grief support, bereavement counseling, and crisis intervention.
  • Support caregivers and family members throughout the hospice journey.
  • Participate in interdisciplinary team meetings and collaborate with nurses, physicians, social workers, hospice aides, volunteers, and community clergy.
  • Coordinate visits with local pastors or faith leaders when requested.
  • Respect and support individuals of all faiths, cultures, and those with no religious affiliation.
  • Document patient care accurately in the electronic medical record (EMR).
  • Maintain confidentiality and follow all hospice, Medicare, and HIPAA regulations.

QualificationsEducation

  • Bachelor's degree required.
  • Master's degree in Divinity, Theology, Ministry, Religious Studies, Pastoral Studies, Counseling, or a related field preferred.

Licensure & Certifications

  • Ordination, endorsement, or recognized standing with a faith organization required.
  • Clinical Pastoral Education (CPE) through an accredited program preferred.
  • Board Certified Chaplain, Certified Hospice Chaplain, Certified Palliative Care Chaplain, or similar certification preferred.

Experience

  • 2–5 years of experience in hospice, hospital chaplaincy, pastoral care, or a related healthcare setting.
  • Experience working with hospice or palliative care patients preferred.
  • Experience in long-term care or skilled nursing facilities is a plus.
  • Experience documenting in an electronic medical record (EMR).
  • Comfortable working with interdisciplinary healthcare teams.
  • Experience supporting patients and families from diverse religious, spiritual, and cultural backgrounds.

Knowledge & Skills

  • Understanding of hospice philosophy and end-of-life care.
  • Knowledge of grief support, bereavement counseling, and the dying process.
  • Understanding of family dynamics, caregiver support, and advance care planning.
  • Ability to provide compassionate spiritual care to people of all faiths and those with no religious affiliation.
  • Training or experience in trauma-informed care and cultural competency preferred.
  • Excellent communication, active listening, and interpersonal skills.
  • Comfortable discussing death, dying, and spiritual or emotional concerns.
  • Strong organizational skills and emotional resilience.
  • Deep compassion, empathy, and respect for every patient's beliefs and values.

Working Conditions

  • Travel to patient homes, assisted living communities, hospitals, and skilled nursing facilities.
  • Valid driver's license, reliable transportation, and proof of insurance required.
  • Ability to perform the physical demands of home visits, including occasional lifting of up to 25 pounds.
