Job Description and Responsibilities
The Case Manager is responsible for direct service, working with the lead case manager in this program and under supervision of the program director. Direct service responsibilities include intake, assessment, completing the care plan as provided by Home Safe or updating it, coordinating among service providers, following up and monitoring, reassessment, and discharge and transitioning care within six-months. The Case Manager uses the agency's electronic health record. The Case Manager also participates in a supervisory collaborative and meets regularly with the supervisor and other case managers in Alameda County for case review and consultation, as well as participating in relevant trainings and collaborative efforts within the agency. Additional responsibilities include advocating with outside agencies on behalf of clients to secure benefits, housing and resources, and maintaining a professional working relationship with providers and other support systems. This direct service position will work with other Felton clinical case managers to allow for increased access to mental health services or other services needed for the clients.
Minimum Qualifications
- A minimum of 2 years of experience working with older adults
- BA/BS Degree in Social Work, Counseling, Psychology, Gerontology or related human services field, or equivalent in related experience
- Strong collaborative skills with team members and community partners
- Solution-focused, fast-paced, person-centered
- Knowledge of Alameda County community resources
- Knowledge of human behavior and performance
