Responsibilities

• Evaluate and develop individualized treatment plans for patients.
  
• Deliver effective, evidence-based physical therapy interventions.
  
• Collaborate closely with occupational and speech therapists, nursing staff, and physicians to optimize patient outcomes.
  
• Document all patient care in accordance with facility and regulatory standards using Net Health.
  
• Provide education and support to patients and their families regarding mobility, strength, and recovery goals.
  

Qualifications

• Licensed Physical Therapist (PT) in the state of Virginia (or eligible for licensure).
  
• Strong interpersonal and communication skills.
  
• Passion for working with geriatric and long-term care populations.
  
• Commitment to teamwork, professional growth, and providing exceptional care.
