Key Responsibilities:
- Assess, plan, implement, and evaluate hospice patient care across multiple settings.
- Develop individualized care plans collaborating with interdisciplinary team members.
- Perform ongoing patient assessments considering physical, psychosocial, and spiritual needs.
- Provide education and support to patients and families, including bereavement follow-up.
- Supervise ancillary staff and handle specialized procedures, such as venous access management.
- Maintain accurate, timely documentation supporting quality care and regulatory compliance.
- Participate in 24/7 on-call rotations, providing coverage during holidays and off-hours.
Required Skills & Qualifications:
- Registered Nurse (RN), licensed in Massachusetts (good standing).
- Associate’s degree in Nursing; Bachelor’s degree preferred.
- Minimum of three years of diverse nursing experience, including at least one year in an acute setting.
- Strong assessment skills and the ability to respond effectively to patient and family needs.
- Knowledge of hospice principles and compassionate care delivery.
- Valid driver’s license with insured vehicle in good condition.
- Excellent communication, teamwork, and organizational skills.
- Ability to work independently, be flexible, and adapt to changing patient needs.
Work Schedule & Compensation:
- Hours: Monday through Friday, Day Shift.
- Schedule options: 20/20 or 24/16 hours weekly; also available in 32 or 40 hours.
- Compensation: $45 - $60 per hour, based on experience and schedule.
- Pay Type: Per Hour.
- Location: Reading, MA.
