Position Summary: This position is responsible for planning, organizing and directing the overall operation of the Social Services department in accordance with current federal, state and local standards, guidelines and regulations governing the Community and as may be directed by the Executive Director to ensure the highest degree of quality resident social services is maintained. Is committed to the values and ethics of the social work profession with emphasis on resident empowerment and self-determination. Advances the value that the resident comes first. The employee values the Community as the residents’ home and works to create attributes of home and models person centered care.

Essential Duties and Responsibilities of the Position: 1. Responsible for the overall management of the social services department, as well as its programs and activities, in accordance with policies, practices and procedures which reflect person centered care, evidence based practice and current rules and regulations. This may include, but is not limited to, management of written policies, practices and procedures which ensure social services staff in meeting the day-to-day needs of residents and the Community. Completes written and oral reports/recommendations as needed regarding the operations of the social services department. Develops methods for coordinating social services with other resident services to ensure the continuity of the resident’s plan of care. 2. Conducts pre-admission services and evaluation of individuals to assess needs. 3. Develops resident social history and assessment by interviewing residents and resident families. Evaluates social and family information and assists in determining plan for social treatment. 4. Manages the selection, evaluation, retention, budgeting and staffing requirements to meet the needs of the residents. 5. Develops social plan of care and participates in implementing and evaluating person centered care for residents in partnership with residents, resident families and team.
