Position: Psychiatric Rehabilitation Coordinator

Family Support Center of Maryland (FSCMD) is a person-centered, family-run psychiatric rehabilitation program located in Baltimore, MD (Lansdowne area). We are looking for a dedicated Psychiatric Rehabilitation Coordinator (PRC) to join our team and growing organization.

We are currently looking to hire a Part-Time PRC and a Full-Time PRC with opportunities for growth. Our ideal candidate is someone who is caring, supportive, and committed to serving our vulnerable community. Are you the right fit for us?

PRC’s are responsible for assuring continuity of care for persons served in the psychiatric rehabilitation program and performing related duties such as providing an individual assistance in procuring entitlements, obtaining and maintaining employment, building a positive support system, and ultimately addressing the needs of each individual and implementing the best plan of action for how we can improve their quality of life.

The care and wellbeing of the individuals we serve at the Family Support Center of Maryland is of the utmost importance and we must ensure that we bring the best of the best in the psychiatric rehabilitation field onto our team.

Qualifications/Requirements

  • The desire and ability to work with and advocate for all populations, including the disabled and vulnerable;
  • An associate degree or bachelors degree; may be working toward a bachelors degree (at minimum);
  • A minimum of two years of PRP-Adult experience (preferred);
  • Must attend at least 40 hours of training in the area of psychiatric rehabilitation prior to providing any direct services with the organization;
  • Must have DLA-20 certification or be willing to obtain DLA-20 certification;
  • Must possess a valid MD state drivers license with proof of insurance and no more than 2 points on drivers license, required;
  • Must be willing to provide persons served with transportation, as needed;

Job Summary

  • Hours: minimum of 20 hours per week
  • Pay range: $17.00-20.00 per hour
  • Part-Time Employee Schedule, at minimum two (2) days during the workweek (Monday-Friday) in the center from 9am-2pm, out in the community as needed other days.
  • Full-Time Employee Schedule, at minimum three (3) days during the workweek (Monday-Friday) in the center from 9am-3pm, out in the community in the afternoons and as needed other days.
  • Participate in the implementation of the individual rehabilitation plan (IRP) with guidance and supervision from the Rehabilitation Specialist.
  • Provide assistance to client in the area of: self-care skills, social skills, independent living skills, cultural interests, medication compliance, health promotion, individual wellness self-management and recovery.
  • Provide assistance with concrete tasks such as getting driver’s license or state ID, gaining employment, finding stable housing and apply for appropriate entitlement programs.
  • Complete progress notes for each client visit using ICANotes Behavioral Health EHR within a 24 to 48 hour time-frame, as well as monthly collaboration progress notes.
  • Submit weekly reports and stay up to date on the company databases.
  • Participate in ongoing on-site and off-site sessions with persons served with a minimum of 6 sessions per month for adults.
  • Collaborate with referring therapist and other treatment team members to ensure the persons served needs are being met.
  • Conduct at least one psychoeducational group per week at the center under supervision of rehabilitation specialist.
  • Participate in 10 hours of ongoing trainings per year in the area of psychiatric rehabilitation.
  • Start and/or end your work day with us in the center, dependent on sessions scheduled with individuals out in the community.
  • Ability to read, understand and follow detailed departmental procedures, and state and federal laws and regulations.

Job Types: Full-time, Part-time

Pay: $17.00 - $20.00 per hour
