Responsibilities

  • Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary.
  • Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration to track health status accurately.
  • Administer medications following prescribed schedules and medical documentation protocols while ensuring compliance with HIPAA regulations.
  • Provide specialized care for clients with dementia, Alzheimer's disease, or other memory-related conditions through tailored memory care strategies.
  • Support clients during hospice or palliative care by offering comfort measures and emotional support in accordance with care plans.
  • Maintain detailed medical records using Electronic Health Record (EHR) systems and document all patient interactions clearly and accurately.
  • Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe home environment to promote overall well-being.
  • Observe patient conditions closely for any changes or concerns related to health or safety and communicate findings promptly to supervising healthcare professionals.
  • Use medical terminology confidently when documenting care activities and communicating with healthcare teams.
  • Assist with medical procedures such as catheter care or other basic nursing tasks as trained and authorized.

Skills

  • Proven experience working in assisted living facilities, nursing homes, or home health environments with a strong understanding of long-term care practices.
  • Familiarity with EMR (Electronic Medical Records) and EHR systems for accurate documentation of patient information.
  • Knowledge of medication administration protocols and ability to follow detailed care plans precisely.
  • Expertise in caring for individuals with developmental disabilities, dementia, Alzheimer’s disease, or memory impairments through specialized techniques like memory care strategies.
  • Ability to perform patient observation effectively by monitoring vital signs and recognizing early signs of health deterioration.
  • Strong communication skills for working collaboratively within multidisciplinary teams while maintaining patient confidentiality per HIPAA standards.
  • Physical stamina to assist patients with mobility needs using equipment such as Hoyer lifts safely and confidently.
  • Experience providing home & community-based care services that foster independence while ensuring safety at all times.
  • CPR certification along with knowledge of emergency response procedures to handle urgent situations efficiently.
  • Flexibility for live-in employment options or scheduled shifts that accommodate client needs around the clock if required.
