Job Overview

The Truancy Case Manager provides community-based case management and supportive intervention services to youth and families experiencing chronic school absenteeism and truancy throughout Montgomery County. This position works collaboratively with families, schools, and community partners to identify barriers to attendance, improve school engagement, and reduce the risk of involvement with the child welfare and juvenile justice systems.

Duties

  • Respond promptly to referrals and initiate contact with youth and families.
  • Conduct intake meetings in homes, schools, the office, or other community settings.
  • Complete required intake documentation, releases, and assessments.
  • Conduct strengths-based, trauma-informed assessments to identify barriers to school attendance and family stability.
  • Develop individualized service plans with youth, caregivers, school staff, and referral sources.
  • Monitor attendance progress and support families in addressing barriers to consistent school attendance.
  • Provide ongoing case management, advocacy, resource coordination, and referrals.
  • Connect families to behavioral health, substance use treatment, housing, food, healthcare, transportation, mentoring, and other community resources.
  • Support caregivers in navigating school attendance processes and community services.
  • Collaborate with school districts, OCY, behavioral health providers, and community partners to coordinate services.
  • Provide supportive interventions focused on problem-solving, conflict resolution, coping skills, goal setting, family communication, and positive youth development.
  • Conduct home visits, school visits, and community-based meetings as needed.
  • Maintain accurate, timely, and confidential case records, including assessments, case notes, service plans, and progress updates.
  • Participate in supervision, case reviews, team meetings, trainings, and program reporting.
  • Provide evening or occasional weekend appointments as needed to meet family needs.

Skills & Qualifications

  • Bachelor's degree in Social Work, Human Services, Psychology, Criminal Justice, Education, or a related field preferred.
  • Experience working with at-risk youth and families required.
  • Knowledge of school attendance, truancy intervention, juvenile diversion, or child welfare systems preferred.
  • Strong motivational interviewing, crisis intervention, and conflict resolution skills.
  • Understanding of child and adolescent development, behavioral health, and trauma-informed care.
  • Excellent communication, organization, and documentation skills.
  • Ability to work independently while collaborating effectively with schools, county agencies, and community partners.
  • Valid driver's license, reliable transportation, and ability to travel throughout Montgomery County.
  • Bilingual (English/Spanish or other languages)

Pay: From $43,000.00 per year
