Family Health Care is seeking a compassionate, team-oriented Integrated Behavioral Health & Substance Use Disorder Therapist (IBH-SUD) to join our Cadillac clinic. This full-time role provides mental health and substance use disorder services within an integrated primary care and Medication-Assisted Treatment (MAT) environment. The clinician will work closely with medical providers, behavioral health staff, and community partners to support patients with co-occurring mental health and SUD needs. This role is ideal for a clinician who values collaborative care, evidence-based practices, and serving rural and underserved communities.
Responsibilities
- Provide integrated behavioral health services embedded within the MAT workflow.
- Conduct SUD and mental health assessments, including SBIRT screening.
- Deliver evidence-based interventions such as Motivational Interviewing (MI) and brief CBT.
- Facilitate 8-10 week SUD group treatment cycles.
- Support MAT engagement, adherence, and relapse-prevention strategies.
- Reduce no-show rates through engagements, reminders, and addressing treatment barriers.
- Provide crisis intervention, care coordination, and warm handoffs to internal and external partners.
- Participate in referral-tracking, follow-up, and quality improvement activities.
Qualifications
- Master's Degree in Social Work, Counseling, or related behavioral health field.
- Active Michigan licensure: LMSW - Clinical, LPC, LPCC, LLP, or equivalent.
- Experience working with SUD, MAT, or co-occurring disorders preferred.
- Training in MI, CBT, SBIRT, or willingness to obtain training.
- Comfort working in fast-paced, team-based primary care settings.
- Strong communication, engagement, and crisis-management skills.
