This is a unique opportunity for a pharmacist passionate about working directly with patients in both community and ambulatory care settings. You’ll collaborate with an interdisciplinary healthcare team optimize therapies, support chronic disease management, and improve outcomes. This position is evenly split between our family medicine clinic and outpatient pharmacy.
