Description:

Summary:

Contributes to the care of the patient by developing pharmacotherapeutic regimens and counseling utilizing pharmacology, pathophysiology, pharmacokinetics, and interpersonal communication skills. Provide clinical pharmacy and drug information services to serve hospital staff and collaborate with physicians. Engages in quality improvement and professional growth to meet hospital, departmental, and personal goals. Compound, package, and dispense medications and IV solutions and preparations as ordered. Assist in the supervision of pharmacy technicians and interns in the operations of the pharmacy. Acts as a resource to students of various health disciplines.

  • Works towards achievement of department-wide pillar goals.

People

  • Assist in the training of pharmacy residents and new pharmacists.
  • Supervise Pharmacy Interns and Technicians to ensure proper training and job performance according to established policy, procedure, and protocols.
  • Work collaboratively with physicians, nurses, and other staff in both central pharmacy and unit-based practice to ensure effective pharmacotherapy.

Service

  • Ensure patients are knowledgeable about new medications and the side effects of all medications that they are taking.
  • Perform provider and patient education in a manner that is service-oriented and consistent with MGHP values.
  • Participate in community outreach as a member of MGHP and the Department of Pharmacy; Appropriately documents community service
  • Carry out specific clinical protocols, such as IV-to-PO conversions and therapeutic substitutions, as defined by Pharmacy and Therapeutics.
  • Demonstrates through leadership and support a "servant" attitude that exemplifies the philosophy, mission, values and vision of MGHP and Trinity-Health

Quality

  • Compound and dispense prescription orders from providers, ensuring patient safety, accuracy, and preventing drug incompatibilities for oral and parental medications.
  • Interact professionally and effectively with team members to ensure a collaborative plan of services. Keep supervisors and employees informed about issues.
  • Maintain required records and automated dispensing cabinet inventories for controlled substances. Helps investigate controlled substance discrepancies.

What you will need:

  • BS degree in Pharmacy. Licensed to practice in the State of Michigan. Advanced degrees (i.e., Doctorate of Pharmacy) and residency training preferred.
  • Demonstrated ability to interact appropriately and effectively with patients, family members, healthcare team members, ancillary departmental staff, medical staff, visitors and the public at large.
  • Demonstrated ability to perform duties in a safe manner, adhering to policies and procedures in the delivery of patient care.
  • Able to identify patient care concerns and problems outside scope of responsibility and initiate appropriate corrective action.
  • Ability to change work priorities in consultation with supervisor as the situation warrants. Able to adapt to changing patient and department needs. Work environment requires considerable flexibility.
  • Articulate and demonstrate commitment to the philosophy, values, mission and vision of MGHP.
  • Demonstrated commitment to hospital pharmacy practice, as demonstrated by recent (within 5 years) employment in hospital practice or advanced clinical pharmacy work.

Physical And Mental Requirements And Working Conditions

  • Primarily involves bending, sitting, squatting, crouching, stooping, walking, standing, and reading.
  • Requires lifting, carrying and/or pushing/pulling weight up to 50 pounds.
  • Is very frequently performing detailed tasks, subject to interruptions, and evolving and changing work priorities.
  • Involves exposure to vapors, fumes and gases, and infectious disease.
  • Able to work variable, split, and additional shifts, including weekends, holidays, and nights.
  • Demonstrate ability to interact appropriately and effectively with patients, family members, healthcare team members, ancillary departmental staff, medical staff, visitors and the public at large.
  • Demonstrate ability to perform duties in a safe manner, adhering to policies and procedures in the delivery of patient care.

  • Articulate and demonstrate commitment to the philosophy, values, mission and vision of Mercy Health
  • Manage patients on anticoagulants by interpreting treatment goal and adjusting anticoagulants as necessary in Mercy Health's anticoagulation clinic.
  • Collaborate with other healthcare providers as well as providing direct patient care.
  • Meet with patients individually to provide extensive drug and disease state education targeting complex disease states such as diabetes, hypertension, dyslipidemia, asthma, COPD, tobacco dependence, and osteoporosis.
  • Work with patients to improve adherence and evaluate medication regimens.
  • Assist in supervision of pharmacy technicians
