Family First Health is seeking a Full-Time Licensed Mental Health Professional to join our Lebanon office as a Behavioral Health Consultant within our integrated primary care team. The Behavioral Health Consultant (BHC) works side-by-side with primary care providers and care team members to deliver brief, evidence-based behavioral health services in a fast-paced medical setting. Using the Primary Care Behavioral Health (PCBH) model, the BHC provides same-day consultations, brief interventions, and support for patients whose behavioral health needs impact their overall health and well-being. This role is ideal for a clinician who values team-based care, enjoys collaborating with medical providers, and is comfortable providing focused, solution-oriented care within the primary care visit structure. Position Overview:
As a Licensed Mental Health Professional, you will work collaboratively as part of a multidisciplinary Primary Care team to address the mental and behavioral health needs of patients. Your role will include:
- Provide same-day behavioral health consultations during primary care visits.
- Conduct brief functional assessments of behavioral health concerns.
- Deliver brief, evidence-based interventions (typically 1–6 visits).
- Address common primary care behavioral health needs including:
- Depression
- Anxiety
- Stress
- Substance use
- Chronic disease self-management
- Sleep concerns
- Health behavior change (medication adherence, diet, exercise)
- Support patients with chronic medical conditions impacted by behavioral health factors.
- Provide crisis assessment and stabilization when needed.
- Work closely with Primary Care Providers (PCPs), nurses, care managers, and other team members.
- Participate in warm hand-offs and curbside consultations.
- Provide real-time recommendations to medical providers.
- Assist with behavioral health triage and care planning.
- Document consultations and interventions in the electronic health record (EHR).
- Maintain documentation that supports integrated, brief care delivery.
- Track patient outcomes and participate in population health management.
- Participate in team huddles and case consultation.
- Provide education to medical staff on behavioral health topics.
- Support development and maintenance of PCBH model fidelity.
Qualifications:
- Licensed in the state of Pennsylvania as an LCSW, LPC, LMFT, or similar credential
- Experience working in a primary care, community health, or integrated care setting preferred
- Demonstrated comfort and clinical experience working with children, especially ages 5–11
- Excellent communication and collaboration skills
- Passion for working in underserved communities
- Bi-lingual (Spanish) is a plus

Working Hours are Monday- Friday between the hours of 8:00am – 6:00pm depending upon individual office need.
