JOB SUMMARY: The Family Preservation Specialist will carry a dedicated case load and provide a range of in-home services to families to improve the relationship between parents and their children. These trainings will address parental behaviors related to home safety, cleanliness, child medical care, and parent child interactions. This individual will maintain a flexible schedule which includes some evenings.
POSITION SPECIFIC DUTIES & RESPONSIBILITIES:
- Conduct comprehensive assessments of clients' physical, psychological, social, and environmental needs.
- Train and educate clients about parenting and life skills.
- Implement individualized care plans in collaboration with clients, families, and other stakeholders that address the unique needs and goals of clients, promoting their connection to their community through tailored support and skill-building activities.
- Coordinate and monitor the delivery of services to ensure timely and appropriate care.
- Advocate for clients' rights and assist them in accessing necessary resources and services.
- Provide emotional and practical support to clients and their families.
- Regularly evaluate clients' progress and adjust care plans as needed.
- Maintain accurate and up-to-date documentation of clients' care, progress, and outcomes.
- Facilitate communication and collaboration among multidisciplinary teams, including healthcare providers, social workers, and therapists.
- Participate in case conferences, team meetings, and other collaborative activities to ensure coordinated care.
- Educate clients and families about available services and resources and assist them in making informed decisions.
- Ensure compliance with all relevant laws, regulations, and standards of practice.
- Participate in continuing education, training, and professional development opportunities to stay current with best practices and trends in case management. Completing certification renewals and all assigned trainings by deadline.
QUALIFICATIONS
EDUCATION:
- Must be a high school graduate or have HS equivalent
EXPERIENCE:
- Experience in the field of mental health, criminal justice, or community based social services required.

CERTIFICATIONS/LICENSES:
- Must possess a valid Driver License and satisfactory driving record to use agency and/or personal automobile to travel to locations other than primary office and to travel to counties outside Tulsa Metro.
- SafeCare Certification
- ODMHSAS Case Manager I Certification, completed within 90 days of eligibility.
- ODMHSAS Case Manager II Certification, completed within 90 days of eligibility.

SAFETY SENSITIVE JOB CLASSIFICATION:
This job is classified as a “safety-sensitive” position as defined by the Oklahoma Medical Marijuana and Patient Protection Act. Due to the “safety-sensitive” classification, an employee in this position would be subject to drug and alcohol testing, including random testing.
