Certified Community Behavioral Health Clinic (CCBHC) provides integrated medical and behavioral health care to clients in an office-based setting.
Position provides advocacy, linkage, referral, engagement with ongoing services with a multidisciplinary team. Coordinating these services to clients in CCBHC program in a client-centered and culturally responsive way. Provides a point of contact and work in partnership with CCBHC staff and partners as part of a multidisciplinary team to provide services to clients and work within a team
- DUTIES AND RESPONSIBILITIES:
- Provide case management services
- Provide skill building to clients, rehab services, to help them manage symptoms of their mental illness
- Work in a team to provide care to clients
- Provide wellness services to clients
- Coordinate medical care
- Provide individual and group services
• Participate in hospital and emergency department discharge processes to transition consumers to a safe community setting according to CCBHC protocols. Ensure timely transfer of medical records, prescriptions, and active follow-up.
• For consumers at risk for suicide, coordinate consent and follow-up within 24 hours and implement a continuing plan for suicide prevention and safety and linking to services.
QUALIFICATIONS: Requires Bachelor’s degree in social work or equivalent education. ODMHSAS CMII certification preferred.
SKILLS AND KNOWLEDGE: Must obtain Case Management II Certification and Behavioral Health Wellness Coach certification upon hire.
TRAVEL AND SPECIAL REQUIREMENTS: Must possess Driver License and use personal automobile for local travel. Must transport clients in personal automobile.
This job is classified as a "safety-sensitive" position as defined by the Oklahoma Medical Marijuana and Patient Protection Act.
