Join the Family and Children's Association (FCA). We are looking for an enthusiastic and organized Case Management Intern to bring their energy and detail-focused skills to our dynamic Clinical department.
Responsibilities
- Provide case management and care coordination services to clients under supervision, including connecting individuals to resources, benefits, housing, healthcare and collaborating with external providers and agencies to ensure continuity of care.
- Review, organize, and update community resource and referral dashboards/folders to ensure accurate and current information.
- Support outreach and engagement efforts through community events, presentations, and program promotion activities.
- Assist with administrative and front desk tasks as needed, including answering phones, greeting clients, scheduling appointments, and managing paperwork.
- Participate in multidisciplinary team meetings, case conferences, and supervision sessions.
- Maintain confidentiality and comply with HIPAA, agency policies, and ethical standards.
- Maintain accurate and timely client records, reports, and documentation in compliance with agency and regulatory requirements.
- Currently enrolled in a Bachelor's or Master's degree program in Social Work, Psychology, Counseling, Human Services, Public Health, or a related field.
- Strong interpersonal, communication, and organizational skills.
- Interest in behavioral health, integrated care, and community-based services with ability to work effectively with individuals and families across the lifespan, including diverse cultural and socioeconomic populations.
