The Opportunity
The Care Coordinator will explore Social Determinants of Health barriers to care and provide case management and health education for individuals and families receiving services in the Sherpa program. The Care Coordinator will communicate with outside providers and assist Sherpa's CRPA staff with management of referrals and complex care coordination as a means of increasing participant access to various resources and services.

Responsibilities
- Complete Social Determinants of Health screenings and provide referrals to assist with barriers and priority needs
- Monitor client progress towards improving access to care through follow-up with participant and service providers as needed.
- Assure the provision of education, risk reduction, and counseling to all clients regarding Mental Health and Physical Health services such as psychotropic medication management and testing and/or treatment for HIV/AIDS, tuberculosis, hepatitis, sexually transmitted diseases, and/or other communicable diseases or co-occurring conditions.
- Ensure entitlements, insurance, and benefits are in place.
- Coordinate with CRPA staff to manage coordination of referrals for resources and transportation, providing warm hand offs and follow-up on outcomes with providers.
- Coordinate with other facilities to ensure the continuity of care as clients transition between care systems.
- Conduct outreach to gather community resources and connections to address participant needs.
