What are the responsibilities and job description for the Nurse Practitioner/Physician Assistant position at Family Care & Wellness?
Family Medicine Advanced Practice Provider | Family Care and Wellness | Allen, Texas
Build a Career Where You Can Truly Make a Difference
Are you an experienced Nurse Practitioner (APRN) or Physician Assistant (PA-C) who believes primary care should be about more than treating illness — that it should center prevention, wellness, and lasting lifestyle change?
Since opening our independent practice in 2020, Family Care and Wellness has intentionally built a different kind of medical practice — one where providers have the time to truly listen, educate, and partner with patients on their journey toward better health. After more than 16 years of practicing medicine, our physician founder set out to build a practice where quality comes before quantity, prevention is prioritized, and every patient leaves feeling heard, respected, and genuinely cared for.
We're looking for an experienced APRN or PA-C to join our collaborative primary care team and help carry that mission forward.
About the Position
As our Nurse Practitioner (APRN) or Physician Assistant (PA-C), you'll maintain an independent patient panel while working closely with our physician to deliver comprehensive outpatient primary care for patients ages 6 through older adulthood.
Our providers average approximately 17 patient visits per day — enough time to provide thoughtful, personalized care and build meaningful relationships, without the rushed pace of high-volume medicine.
Every patient should leave feeling:
- Heard
- Respected
- Confident in their care plan
- Encouraged about their health
- Genuinely cared for
What You'll Do
As a APRN or PA-C on our team, you'll provide independent, high-quality medical care under the general supervision of our physician, including:
- Obtain detailed patient histories and perform physical examinations; assess and treat acute and chronic medical conditions and provide direct, hands-on patient care
- Perform annual wellness visits and preventive screenings
- Counsel patients on nutrition, exercise, sleep, stress management, and other sustainable lifestyle habits
- Consult with the clinic supervising physician as needed.
- Follow safety, environmental, and infection-control best practices
- Help guide and coordinate the work of clinic support staff
- Document patient visits, progress notes, and treatment plans accurately and in a timely manner
- Build long-term, trusting relationships with patients and provide meaningful health education to patients and families
- Recognize when a patient would benefit from additional lifestyle coaching or wellness support beyond the traditional visit, and thoughtfully connect them with resources available through Wellness Lifestyle Partners, when appropriate
- Stay current through continuing education, professional literature, and in-service training
Through Wellness Lifestyle Partners, we connect appropriate patients with evidence-based wellness coaching and additional resources that complement — not replace — their primary care, so every patient gets the level of support they need to reach their healthiest life.

Our ideal Nurse Practitioner or Physician Assistant:
- Values quality patient care over high patient volume
- Believes prevention is the foundation of excellent primary care

- Is open to, or already interested in, integrative, functional, holistic, or lifestyle medicine
