Role Description
This is a full-time role located in Bullhead City, AZ, Lake Havasu & Kingman with opportunities for some remote work. As a Nurse at Family Care Home Health & Hospice, you will provide direct patient care, conduct patient assessments, and develop care plans tailored to individual needs. Responsibilities include administering medications, monitoring patient progress, offering emotional support, and collaborating with a multidisciplinary team to ensure the highest level of care. Additional duties involve educating patients and their families about treatments, health maintenance, and promoting overall well-being.
Qualifications
- Competence in patient assessments, care planning, and administering treatments
- Strong communication and interpersonal skills to effectively interact with patients, families, and the healthcare team
- Understanding of hospice and palliative care principles (preferred for Arizona-based roles)
- Proficiency with patient education and advocacy to promote long-term wellness
- Ability to manage time effectively and perform in both in-person and remote work environments
- Active Registered Nurse licensure in the state of Arizona and/or Nevada
- Current CPR certification and knowledge of healthcare regulatory standards
- Experience in home health or hospice care is a plus
