Position Summary
The MDS Nurse Manager is responsible for coordinating and completing the MDS assessment process, ensuring accuracy and compliance with state and federal regulations, and leading the interdisciplinary care planning process.

Requirements

• Must be a Licensed RN or LPN in the state of Illinois

Qualifications

• Previous experience in an MDS and/or Care Planning role required
• Strong written, verbal, and interpersonal communication skills
• Ability to work Monday through Friday, 8:00 AM – 4:30 PM
• Participation in an on-call rotation

Wages

• RN: $33.00 – $35.00/hour
• LPN: $29.00 – $31.00/hour
