Job Overview
We are seeking a dedicated and detail-oriented RN/LPN - MDS Coordinator to join our healthcare team. In this vital role, you will oversee the comprehensive assessment and documentation of residents’ clinical needs, ensuring compliance with regulatory standards and optimizing patient care quality. Your expertise will facilitate accurate Medicare and Medicaid billing, support clinical documentation improvement initiatives, and coordinate interdisciplinary efforts to deliver exceptional resident outcomes. Responsibilities
- Record patients’ medical history accurately
- Coordinate & complete MDS assessments in accordance with federal & state regulations.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care plans are developed & implemented.
- Maintain up-to-date knowledge of NCQA standards and regulatory changes affecting MDS processes.
- Facilitate discharge planning by collaborating with case management teams to ensure smooth transitions for patients.
- Provide training and support to nursing staff on MDS-related processes & documentation requirements & Direct staff as necessary
- Provide emotional & psychological support when needed
- Communicate with patients’ family or friends to provide advice, comfort, & release instructions
- On-Call rotation every 3rd weekend with Nursing Administration
Skills
- Proven experience as RN or similar role
- Strong knowledge of medical terminology, anatomy, physiology, & critical care practices.
- Experience with the MDS Assessment federal & state regulations.
- Ability to undertake a variety of diverse care tasks and work with a multidisciplinary team
- Familiarity with hospital health, safety & sanitation standards and procedures
- Understanding of confidentiality obligations & nursing best practices
- Excellent communication & interpersonal skills
- Compassionate & able to handle stress
- BLS certification preferred
- A true passion for caring for the elderly
