Key Responsibilities
- Deliver patient treatment based on physician orders, including one-on-one, supervised, and group sessions.
- Screen and evaluate patients/residents to develop appropriate care plans.
- Participate in clinical program development, staff training, and quality improvement efforts.
- Provide consultation and training to patients, families, and caregivers.
- Document activities accurately and timely, ensuring compliance with reimbursement regulations.
- Communicate effectively with patients, families, staff, and supervisors to promote positive relationships.
- Ensure service and operational excellence in all activities to meet quality standards.
- Ability to exert 50-100 pounds of force for resident transfers.
- Minimum of two years relevant experience, preferably in long-term care.
- Excellent verbal and written communication skills.
- Strong interpersonal skills to build relationships with residents and staff.
- Genuine interest in working with elderly and/or disabled individuals.
- Bachelor's degree in Physical Therapy from an accredited program.
- Current state license as a Physical Therapist.
