We Fairview Health Services are looking for a driven Team Coordinator to be our Phones Specialist at the Fairview Clinic – Bass Lake in Maple Grove, Minnesota! The Bass Lake location is small yet dedicated clinic in the M Health Fairview system. The staff is hard working and is empowered to view everything as "our" work to accomplish all goals. We also make sure everyone is cross-trained, while still working in scope, they are able to effectively support the team.
As an integral member of the primary care team, the team coordinator (TC) contributes to overall quality of care and patient experience via phone calls and electronic messaging. Team Coordinators perform high-level functions to support the needs of the healthcare team and patients/support network between office visits to ensure patient needs are successfully addressed. They will deliver direct support to patients on the coordination of medical information requests and updates, as well as referrals and appointment scheduling. The TC will use a variety of communication tools and systems in their work to support patients and their care team. Additionally, we ask the TC's to maintain complete, timely and accurate documentation for each patient interaction. They will also continually look for opportunities we can improve processes and workflow.
- Coordinate patient and provider schedules for assigned areas.
- Complete patient registration and check-in process.
- Verifies patient demographic/employer and insurance information.
- Performs telephone message taking and directs communication in a timely and professional fashion.
- Handles MyChart requests including patient sign-up, responding to patient messages and resetting passwords.
- Multi-task and prioritize appropriately.
Required Qualifications
- Successful completion of formal medical training program such as MA, HUC, NA or Medical Secretary, or min. of 1 year comparable experience in a healthcare setting
Preferred Qualifications
- 2 or more years’ experience working in a medical setting
