Holistic Health Coach - FPS

Serves as holistic health and wellness coach for Fairview Specialty Pharmacy patients. This position supports patients with complex medical conditions who may need additional support to navigate and connect to various services.

Location: Seven Lakes Pharmacy - 3650 Victoria St N Shoreview

Schedule: Flexible hours during 8:00AM-4:00PM M-F

Responsbilities:

• Conducts one-on-one well-being intake and consultation sessions with Fairview Specialty Pharmacy patients
• Develops patient relationships that facilitate improvement in holistic wellness by encouraging self-discovery, personal responsibility, and increased self-efficacy
• Determines the patient’s desired areas of focus, encourages goal setting, and additional resources for:
  • Lifestyle changes
  • Stress management strategies
  • Emotional and/or spiritual support resources
  • Loss or grief counseling resources
• Foster healthy grieving in the wake of primary and secondary health losses and difficult life transitions
• Encourage and facilitate direct communication between patients and medical caregivers or family members regarding health-related matters
• Assists in identification and guidance to overcome barriers to healthy coping
  • Partner with patients who wish to make positive lifestyle changes including smoking cessation, healthy diet, increased exercise.
  • Refers patients to additional Fairview and external resources for support
• Assessment, Planning, Evaluation: Develops, coordinates, and implements an individualized plan of care to achieve desired outcomes. Evaluates progress towards goals and outcomes identifying opportunities for improvement and change
• Coaching: Ability to provide guidance and feedback to help a patient strengthen their knowledge, skills, and abilities to accomplish personal goals related to their health conditions

Required Qualifications

• Bachelor of Arts in, counseling, theology, human services or health coaching
• Preferred additional graduate-level coursework on integrative medicine, health coaching/counseling and other wellness-related topics.
• 3 years experience within the healthcare field providing counseling and one on one patient support.

Preferred Qualifications

• Master of Arts in integrative health and wellbeing coaching, social work, or psychology
• Health Coach Certification NBHWC
• Proposal for FSSP Holistic Health Coach

An individual's pay rate within the posted range may be determined by various factors, including skills, knowledge, relevant education, experience, and market conditions. Additionally, our organization prioritizes pay equity and considers internal team equity when making any offer. Hiring at the maximum of the range is not typical. If your role is eligible for a sign-on bonus, the bonus program that is approved and in place at the time of offer, is what will be honored.
