PROJECT COORDINATOR

GRANT SUMMARY

The goal of HOPE is to improve mental health outcomes for children and youth at risk for or with serious emotional disturbance and to bring new hope through its comprehensive and early intervention approach for the children, youth, and their families.

POSITION SUMMARY: 

This position is responsible for coordinating HOPE services among the Behavioral Health Community Coalition (BHCC) and four (4) behavioral health service units: Screening, Brief Intervention, and Referral to Treatment (SBIRT), Assessment, Children, Youth and Young Adult Services (YYAS), and Suicide Prevention.

JOB DUTIES

• Conducts Screening, Brief Intervention, and Referral to Treatment (SBIRT) with youth referred to HOPE using empathy and engendering optimistic empowerment for consumers; using evidence-based practices identified by the Hope for our Children or FNA. 
• Will provide a warm handoff for any consumer qualifying for HOPE with a Family Peer Specialist who will provide crisis peer support with the family until youth is admitted to day treatment. 
• Ensures that a chart is created on the electronic health record for new consumers in one business day. 
• Tracks referral times. 
• Streamlines care to ensure smooth service delivery by meeting monthly with HOPE staff and allied service units.
• Participates in monthly meetings to assess progress and address the challenges of the consumer’s experience.
• Participates in continuous quality improvement and data-driven decision-making processes to assess program efficacy and drive quality improvement of service flow.
• Assesses service flow annually and reports findings to supervisor. 
• Ensures adherence to data collection requirements and enters accurate and timely data into the Electronic Health Record. 
• Attends mandatory position training that provides training in risk factors as designated by the Project Director and/or Clinical Supervisor.
• Attends all required staff meetings.
• Performs other job-related duties as assigned.

NECESSARY KNOWLEDGE, SKILLS, AND ABILITYs

• Ability to report to work on time.
• Knowledge of substance abuse and mental health disorders to use the appropriate skills when working with consumers who have mental health or substance use disorders.
• Skill working successfully with the Alaska Native/American Indian population and experience providing culturally specific programming. Understanding of and sensitivity to diverse cultures and lifestyles.
• Skill with operating computers, including the applications of Word and Excel, with the ability to accurately enter information into reporting formats. 
• Above average communication and people skills with the ability to accurately follow written and verbal instructions.
• Knowledge of how to best assist possible escalated individuals during the data collection process.
• Ability to protect confidentiality is essential; HIPAA & 42 CFR compliant.
• Knowledge and ability to interpret/understand billing procedures.
• Skill in managing multiple priorities and tasks concurrently and meeting their deadlines.
• Skill in establishing and maintaining cooperative working relationships with colleagues, supervisors, agencies, and consumers.
• Ability to work independently as well as with teams.

• High School Diploma or equivalent.
• Experience in System of Care (SOC) services.
• One (1) year of experience working with AN/AI population. 
• One (1) year of experience working in the mental health field.

• Experience working with infants and toddlers.

• Experience providing culturally specific services.
  • Bachelor’s degree in behavioral health or a related field.
