PROJECT COORDINATOR (Project Liaison)

GRANT SUMMARY

The goal of the Athabascan Suicide Response Project is to reduce confrontations with law enforcement and connect people in crisis to the right help, right away. This project is framed around the concepts of community, family, and culture and integrates elements of each into its suicide response following referral. The overarching goal is to develop the infrastructure to support the 988 Initiative to increase the capacity of the behavioral health community to implement, sustain, and improve effective culturally responsive suicide 988 crisis calls and response.

POSITION SUMMARY 

This position is responsible for tracking the support service of the Athabascan Suicide Recovery Project from referral through discharge of individual consumers. This position works collaboratively with the 988 Subcommittee to fulfill duties of the project by partnering with Lifeline 988 crisis centers, develops staff response, includes peer support, to establish follow-up contact protocols for American Indian and Alaska Native (AI/AN) consumers for and after crisis encounters.

JOB DUTIES

• Tracks referral times and supports coordination of timely follow-up services. 


• Participates in intake meetings with project staff and new consumers upon entry into the project.


• Provides peer support services to project consumers in treatment or other supportive services as assigned or requested by the Program Director or requested by Project staff. 


• Advocates for consumer needs and perspectives during care coordination and planning discussions.


• Collaborate with the marketing team to support community education and awareness regarding the transition from 911 to 988 services. 


• Coordinates with State 988 Crisis Center and partner organizations to support referrals to culturally appropriate services.


• Coordinates outreach efforts with Fairbanks community organizations, partners, and businesses to support the development and implementation of Memorandums of Understanding (MOUs) and Memorandums of Agreement (MOAs) related to 988 collaboration, referral pathways, and community response initiatives.


• Serves as a liaison between the 988 project and community stakeholders to strengthen partnerships, improve communication, and support coordinated service delivery across the Fairbanks region.


• Attends Behavioral Health Community Coalition (BHCC) meetings in a collaborative capacity.


• Supports coordination between project staff and allied service providers to promote continuity of care and smooth service delivery.


• Assists with monitoring consumer progress and identifying service delivery challenges through regular communication with staff and partners.


• Participates in continuous quality improvement and data-informed processes to support program effectiveness and service delivery.


• Assists with annual reviews of service flow processes and provides observations and recommendations to the Project Manager.


• Maintains adherence to data collection requirements and enters accurate and timely data into the Electronic Health Record (EHR). 


• Attends mandatory position training related to risk factors, crisis response, and project implementation as designated by the Project Manager and / or Clinical Supervisor.


• Attend all required staff meetings.


• Performs other job-related duties as assigned.

NECESSARY KNOWLEDGE, SKILLS, AND ABILITY

• Ability to report to work in a timely manner.


• Knowledge of substance abuse and mental health; and appropriate skills in working with consumers with Mental Health/Substance Use disorders.


• Skill in working successfully with the Alaska Native/American Indian population and experience providing culturally specific programming; understanding of and sensitivity to diverse cultures and lifestyles.


• Skill in operating/using computers and using Windows, Word, and Excel with ability to accurately input information into reporting formats. 


• Above average communication and people skills with the ability to accurately follow written and verbal instructions.


• Knowledge on how to best assist possible escalated individuals during data collection process.


• Ability to protect confidentiality is essential; HIPAA & 42 CFR compliant.


• Knowledge and ability to interpret/understand billing procedures.


• Skill in managing multiple priorities and tasks concurrently and meeting their deadlines.


• Skill in establishing and maintaining cooperative working relationships with colleagues, supervisors, agencies, and consumers.


• Ability to work independently as well as with teams.

• High School diploma or GED.


• One (1) years’ experience working with AN/AI population and experience providing culturally specific services.


• One (1) years’ experience in behavioral health.


• One (1) years’ experience working with electronic health records. 

• Associate’s degree in behavioral health or related field.


• Experience working with transition-aged youth and young adults.


• Lived experience with mental illness and/or using mental health services.
