Intellectual Disabilities Division
Strives to help individuals with an intellectual disability, autism, or developmental delay achieve a more meaningful and productive life. ECCM works closely with state and local government agencies as well as direct service providers as the “center of the system”.
Following the “Everyday Lives” approach, our supports coordinators will help individuals navigate their way through a system designed to meet their needs in a community setting, often in their own homes (75% community setting & 25% from home office). Supports coordinators will work with individuals and families to identify desired life goals and work to find service providers to achieve these outcomes.
Supports Coordinator
- Development and maintenance of an Individual Support Plan (ISP).
- Identifying needed services and locating qualified providers.
- Locating and coordinating funding for needed services.
- Monitoring services to ensure delivery and progress toward goals.
- Ensuring consumer health and safety.
Looking for the following majors:
Psychology, Criminal Justice, Sociology, Social Work, Gerontology
- or - Bachelor's degree and a combined 12 credits in any of the above majors.
- or - Associate's degree and two years' experience in a position performing paraprofessional case management functions such as: interviewing, providing information, referring to community resources, and determining or verifying eligibility for services.
