Job Overview
We are seeking a compassionate and dedicated Hospice Admissions Nurse with at least one year of previous hospice experience to join our dynamic team. Duties
- Conduct comprehensive patient assessments upon admission, including vital signs, medical history review, and physiological evaluations.
- Coordinate with physicians, social workers, and other healthcare professionals to develop personalized care plans aligned with patient needs and preferences.
- Manage all aspects of medical documentation within electronic health record (EHR) systems, ensuring accuracy and compliance with HIPAA regulations.
- Educate patients and families about hospice services, medication management, symptom control, and advance directives to promote informed decision-making.
- Facilitate communication between the interdisciplinary team to ensure seamless transitions into hospice care.
- Monitor patient comfort levels, manage acute pain or symptom crises using appropriate interventions, and adjust care plans accordingly.
- Assist with discharge planning when necessary, including coordination with long-term care facilities or home health agencies.
- Coordinates admissions of hospice patients by making skilled assessments of patients initial physical, psychosocial and spiritual needs.
- Coordinates the establishment of the initial plan of care with the attending physician, medical director and the interdisciplinary team.
- Provides skilled interventions and instructions to patient and caregivers to provide optimum symptom control and patient comfort under the direction of physician.
- Functions as a liaison between disciplines and facilities during the admission process.
- Develops, prepares and maintains individualized patient care progress records with accuracy, timeliness and according to policies. Submits accurate documentation per care center policy. Records pain/symptom management changes/outcomes as appropriate.
- Communicates significant findings, problems, changes in condition or environment to the clinical manager/ physician and other members of the hospice care team. Communicates with the physician regarding the patients' needs and reports changes in the patient's condition; obtains physician orders as required.
- Performs on-call responsibilities and on-call services to patients/families as assigned.
- Performs other duties as assigned.
Qualifications
- Minimum of two years of hospice nursing experience preferred; experience in critical care, ICU, or emergency medicine is highly valued.
- Proficiency with EMR/EHR systems; familiarity with ICD-10 coding and medical documentation standards.
- Strong assessment skills in physiology, anatomy, and pharmacology related to palliative medicine and pain management.
- Ability to handle sensitive situations with empathy while maintaining professional boundaries; experience working with people with developmental disabilities or in geriatrics is advantageous.
