Job Overview
We are seeking a compassionate and dedicated Hospice Admissions Nurse with at least one year of previous hospice experience to join our dynamic team. In this vital role, you will be the first point of contact for patients and families entering hospice care, ensuring a smooth and respectful admission process. Duties

  • Conduct comprehensive patient assessments upon admission, including vital signs, medical history review, and physiological evaluations.
  • Coordinate with physicians, social workers, and other healthcare professionals to develop personalized care plans aligned with patient needs and preferences.
  • Manage all aspects of medical documentation within electronic health record (EHR) systems, ensuring accuracy and compliance with HIPAA regulations.
  • Educate patients and families about hospice services, medication management, symptom control, and advance directives to promote informed decision-making.
  • Facilitate communication between the interdisciplinary team to ensure seamless transitions into hospice care.
  • Monitor patient comfort levels, manage acute pain or symptom crises using appropriate interventions, and adjust care plans accordingly.
  • Assist with discharge planning when necessary, including coordination with long-term care facilities or home health agencies.
  • Coordinates admissions of hospice patients by making skilled assessments of patients initial physical, psychosocial and spiritual needs.
  • Coordinates the establishment of the initial plan of care with the attending physician, medical director and the interdisciplinary team.
  • Provides skilled interventions and instructions to patient and caregivers to provide optimum symptom control and patient comfort under the direction of physician.
  • Functions as a liaison between disciplines and facilities during the admission process.
  • Develops, prepares and maintains individualized patient care progress records with accuracy, timeliness and according to policies. Submits accurate documentation per care center policy. Records pain/symptom management changes/outcomes as appropriate.
  • Communicates significant findings, problems, changes in condition or environment to the clinical manager/ physician and other members of the hospice care team. Communicates with the physician regarding the patients' needs and reports changes in the patient's condition; obtains physician orders as required.
  • Performs on-call responsibilities and on-call services to patients/families as assigned.
  • Performs other duties as assigned.

Qualifications

  • Minimum of two years of hospice nursing experience preferred; experience in critical care, ICU, or emergency medicine is highly valued.
  • Proficiency with EMR/EHR systems; familiarity with ICD-10 coding and medical documentation standards.
  • Strong assessment skills in physiology, anatomy, and pharmacology related to palliative medicine and pain management.
  • Ability to handle sensitive situations with empathy while maintaining professional boundaries; experience working with people with developmental disabilities or in geriatrics is advantageous.
  • Current nursing license in good standing; BLS certification required; additional certifications such as hospice & palliative medicine are a plus. Join us to provide compassionate end-of-life care that truly makes a difference!
