Exodus Recovery, Inc is looking to hire a full-time Care Manager for our CalAim ECM Program located in San Diego, CA.
POSITION SUMMARY:
Exodus Recovery, Inc. (Exodus) shall employ individuals to provide high-touch, community based, in person care management services, coordinating all primary, acute, behavioral, oral and long-term services and supports for members in the ECM and CS Programs. The Lead Care Manager (LCM) serves as the primary point of contact for the Member, Member’s family, Authorized Representative (AR), caregiver or other authorized support person(s), as appropriate. The LCM is the point of contact for the multi-disciplinary team and MCP personnel. LCM’s utilize their experience to enhance recovery efforts of members, actively promote understanding of member’s point of view and preferences, and fulfills duties related to supportive counseling, rehabilitation, and collateral interactions with natural supports.
MINIMUM QUALIFICATIONS:
- Two years’ experience working with individuals with complex conditions which may include: chronic physical health issues, mental illness, substance use disorders, homelessness, justice involvement and the frequent use of multiple health systems. Case Management experience preferred.
- Valid CA driver’s license and good driving record is required, if applicable for the program. Must be 21 years or over to drive.
- Bachelor or Master's degree preferred.
- Previous training and experience preferred.
- Bilingual in Arabic, Farsi, and Spanish speakers preferred.
Commensurate with experience.

Pay: $25.00 - $28.00 per hour
