Exodus PCS is a fast-growing and highly regarded provider of the Relocation Service Coordination and Moving Home Minnesota programs. We assist residents of nursing facilities in locating community housing options, including assisted living, foster care, and independent living.

We offer paid training to help Case Managers develop the skills needed to succeed and navigate relocation and community programs. Most work is completed remotely and onsite, offering flexibility and a self-driven, rewarding role in human services.
Key Responsibilities:
- Assist clients in locating and securing appropriate housing options in the community.
- Coordinate and collaborate with service providers, community resources, and families to develop individualized transition plans.
- Navigate and manage community and waiver programs, including CADI and Elderly Waiver.
- Support clients through discharge planning, housing coordination, and case management.
- Consistent communication with support team and completion of case documentation
- Maintain accurate client records using Microsoft 365 and Microsoft Teams.
- Provide assistance during discharge days, including light lifting (up to 25 lbs.).
Qualifications:
- Bachelor’s Degree in Social Work, Psychology, Sociology, Human Services, Nursing, or a related field.
- At least two years of experience in human services, social services, or healthcare.
- Knowledge of community resources, waiver programs, and county applications.
- Experience with medical assistance, discharge planning, and housing coordination.
- Clearance to work in human services through a DHS background check.
- Reliable vehicle with valid driver’s license and insurance.
- Proficiency with Microsoft 365 and Teams.
Location:
Exodus PCS is based in Burnsville, MN. The first few weeks of training will take place in the main office. After training, work is hybrid, with some being completed remotely from home and some in the community on client visits.
